I understand that driving a vehicle owned, leased, or rented by Forty Five Enterprises, LLC (“Company”) is a requirement of the position I am being considered for and that having and maintaining a satisfactory driving record may be a condition of my employment. I agree to allow the Company or its agent check my driving record prior to hire and to check it periodically thereafter. For purposes of determining insurability under the Corporate Automobile Insurance Coverage, the Company is making application for and/or determining driver safety or other use permissible under the provisions of the Driver’s Privacy Protection Act of 1994 (Public Law 103-222, Title XXX, Section 3000002(a)). I understand that the Company will use this information for employment purposes only and not furnish this information to a third party without my written consent. I hereby authorize Forty Five Enterprises, LLC to conduct a criminal background check. I also am aware that records of arrests or pending charges and/or convictions are not an absolute bar to employment. Such information will be used to determine whether the results of the criminal background check reasonably bear on my trustworthiness or my ability to perform the duties of my position in a safe manner. I agree to release Forty Five Enterprises, LLC, its agents, attorneys, employees, and those who supplied the information from any liability for any damage which may result from furnishing the requested information or my failure to be hired for the position for which I am applying.
